
Sarah Saari, DNP, AGACNP is a board-certified Adult-Gerontology Acute Care Nurse Practitioner with extensive experience caring for adults with complex medical and behavioral health needs. She received her BA in Nursing from Luther College and her DNP from the University of Wisconsin-Madison. Her background includes hospital-based care managing complex medical conditions including substance use disorders, as well as experience in community behavioral health, including both a withdrawal management facility and outpatient clinic.
She provides compassionate, evidence-based, and nonjudgmental care for individuals with substance use disorders and co-occurring mental health and medical conditions. Her approach is grounded in harm reduction, patient-centered care, and collaboration, meeting patients where they are and supporting their individual goals for recovery and improved health.
Sarah provides evaluation and treatment for substance use disorders, including opioid and alcohol use disorders, medication management, and connection to behavioral health and community resources. When additional services are needed, she works collaboratively with psychiatry providers, therapists, and other community partners to help patients access the appropriate level of care.
She is committed to creating a welcoming environment where patients feel respected, supported, and empowered to make meaningful progress toward better health and long-term recovery and she is excited to be part of the HealthyMinds team.
